To make an appointment, please call 070 306 5100 and select the option for the Polyclinic.
Phone lines are open Monday to Friday, 08:00–13:00 and 14:00–16:00.
You can also email us at info@relo-reunitegroup.com.
If you are a registered IHCH patient, kindly include your full name and date of birth in your email to help us locate your file.

Referral Information

  • Dutch insurance holders: A referral letter from your GP/Family Doctor is required for your specialist consultation fees to be (partially) covered. Without a referral, you can still book an appointment, but reimbursement may not be possible.
  • International/Private insurance holders: You likely do not need a referral. However, check directly with your insurance provider if you are unsure.
  • Patients without insurance: You are welcome to book an appointment but will be asked to pay consultation fees directly after your visit.
